Diagnosis for eligibility includes:
Diagnosis of Intellectual Disability prior to the age of 22. Examples include: (Autism, Cerebral Palsy, Neurological Impairment)
OMH
Diagnosis for eligibility includes:
Severe and Persistent Mental Illness (SPMI) as a primary diagnosis (Axis 1)
DOH
Diagnosis for eligibility includes:
Diagnosis of Traumatic Brain Injury
Must be between the age of 18 – 64 upon application to TBI Waiver
Be a recipient of Medicaid
