Community Habilitation Services

Community Habilitation is a program that provides one on one, direct support tailored to the persons interests. These services are to help people with disabilities obtain, improve and retain the skills needed to live as independently as possible.

Overview

Family Residences and Essential Enterprises, Inc. (FREE) is a leader of providing community services and offers Community Habilitation services to adults with intellectual or developmental disabilities. The guidance provided by Community Habilitation are intended to cultivate social skills, self-help and skills necessary to live successfully in the community. Our Community Habilitation team works with the people we support and if applicable, their family to ensure activities based on their goals, needs, and interests.

Goals are tailored for each individual and may include:

  • Problem Solving Skills
  • Decision Making Abilities
  • Improving Social Skills
  • Anger Management
  • Independent Living Skills

Community Habilitation Services

  • Have a Care Coordinator and be enrolled in Waiver Services
  • May either live in the community, with a family member, in an OPWDD certified setting, or in a Community Residence
